U.S. passport application document checklist

Prepare the documents that match your application route and verify current forms, fees, photos, and submission instructions.

Use this checklist after choosing the correct form

The documents differ between Form DS-11, online renewal, and Form DS-82. Confirm your form first, then use the sections that apply.

For an in-person DS-11 application

  • Completed Form DS-11, unsigned until instructed.
  • Original or certified U.S. citizenship evidence.
  • Photocopy of the citizenship evidence when required by the instructions.
  • Physical photo ID plus front-and-back photocopy.
  • One recent compliant color photograph.
  • Application fee and execution fee in the payment forms accepted at that location.

For renewal by mail

  • Current Form DS-82, completed and signed.
  • Your most recent eligible passport.
  • One compliant photo attached as instructed.
  • Correct fee payment.
  • Certified name-change document when applicable.

For online renewal

  • Your passport details and the passport in your possession.
  • A digital photo that meets the online specifications.
  • An email address and the required online account.
  • Electronic payment method.
  • No international travel during the processing window.

Before submitting

  • Check the current form edition and instructions.
  • Use the official fee calculator rather than an old fee chart.
  • Check current processing times and add mailing time.
  • Make copies of everything you are allowed to retain.
  • Use only the current mailing address or authorized acceptance facility.
