Use SSA’s name-change service. Some people can complete the request online; others must submit Form SS-5 and show evidence of identity, the new legal name, and the name-change event.
Documents you may need
- Evidence of identity.
- A marriage document, divorce decree, court order, or other acceptable proof of the legal name-change event.
- Evidence of U.S. citizenship or lawful noncitizen status when SSA requires it.
SSA accepts original documents or copies certified by the issuing agency, not ordinary photocopies or notarized copies.
Steps
- Use the official SSA name-change page and answer the questions.
- Complete the online path if offered for your situation.
- Otherwise complete the current Form SS-5 and arrange the required office step.
- Review the corrected card when it arrives and contact SSA promptly if the record is wrong.
