What to prepare

- Original or certified evidence of U.S. citizenship and the required photocopy.
- A physical photo ID and a photocopy of the front and back on white 8.5-by-11-inch paper.
- One compliant color passport photo.
- Payment for the application fee and the separate execution fee using payment methods accepted by the facility.
